Trump names Casey Means as his new pick for U.S. surgeon general

  • President Donald Trump nominated Dr. Casey Means, a functional medicine physician, for U.S. surgeon general – replacing initial nominee Dr. Janette Nesheiwat just before her Senate hearing. The move signals a shift toward metabolic health and disease prevention.
  • Means’ nomination aligns with Health Secretary Robert F. Kennedy Jr.’s “Make America Healthy Again” (MAHA) agenda, focusing on nutrition reform, reducing pharmaceuticals, and combating processed foods and environmental toxins.
  • A critic of ultra-processed foods and the profit-driven healthcare system, Means co-founded the Levels metabolic health app and promotes root-cause medicine over conventional symptom management.
  • Means’ brother, Calley Means, advises Kennedy, reinforcing the administration’s metabolic health focus. Nesheiwat will remain at HHS while her brother-in-law, Mike Waltz, is Trump’s UN ambassador nominee.
  • If confirmed, Means may challenge dietary guidelines and pharmaceutical norms, framing chronic disease as a policy failure. Her nomination sparks debate over healthcare reform, pitting preventive approaches against traditional medicine.

President Donald Trump has nominated functional medicine physician Dr. Casey Means as his choice for the position of U.S. surgeon general.

The chief executive made the announcement Wednesday, May 7, signaling a pivot toward metabolic health and disease prevention. His nomination of Means came a day before his initial nominee, Dr. Janette Nesheiwat, was set to face a Senate confirmation hearing.

The nomination of Means, a vocal critic of processed foods and environmental toxins, aligns with Health Secretary Robert F. Kennedy Jr.’s mission to overhaul the nation’s food supply and reduce reliance on pharmaceuticals – Make America Healthy Again (MAHA). She has long argued that America’s chronic disease epidemic stems from poor nutrition and a profit-driven medical system.

Trump praised Means in a Truth Social post, calling her credentials “impeccable” and predicting she could become “one of the finest surgeon generals in U.S. history.” The role, often dubbed “America’s doctor,” oversees the U.S. Public Health Service Commissioned Corps and has historically been a platform for issuing national health advisories.

Means’ nomination reflects a broader ideological shift within Trump’s health policy team. Her brother Calley already serves as a senior adviser to Kennedy, further solidifying the administration’s focus on metabolic health as a cornerstone of its MAHA agenda.

Means’ professional journey underscores her break from traditional medicine. After leaving a surgical residency in 2017, she turned to functional medicine – a field emphasizing root-cause prevention over symptom management – and co-founded Levels, a health-tech app tracking metabolic responses to food. Her advocacy centers on eliminating ultra-processed foods from schools and reducing environmental toxins – positions that have drawn both fervent support and criticism. Means’ nomination sparks debate: Will she challenge Big Pharma?

In a 2024 interview with the Epoch Times, she argued that the U.S. healthcare system is “set up for poor health.” Means continued: “I believe Americans know that something is not right when it comes to the health trends of children, adults and the elderly”

It’s not the end of the road for Nesheiwat, however, as Trump confirmed that she will continue to work at the Department of Health and Human Services. Incidentally, Nesheiwat is the sister-in-law of Mike Waltz, former national security advisor and currently Trump’s nominee as U.S. ambassador to the United Nations.

The surgeon general’s office has historically wielded influence through high-profile warnings, from the dangers of smoking to the opioid crisis. If confirmed, Means would likely use the position to challenge dietary guidelines and pharmaceutical norms, framing chronic illness as a policy failure rather than an inevitability. Her nomination signals a defining battle over the future of American healthcare – one where metabolic health takes center stage, and the very definition of prevention is up for debate.

The question remains: Will Means’ unorthodox approach gain traction in a divided political landscape, or deepen the rift between conventional medicine and the growing movement demanding radical change? For now, her rise marks a bold bet that America’s health crisis requires not just new solutions, but a fundamentally new paradigm.
